Italy, France and Germany suspend AstraZeneca COVID-19 vaccine

Germany, France, Italy and Spain have suspended the use of AstraZeneca’s COVID-19 vaccine after several reports of blood clots in people who received the shot in Europe.

The flurry of suspensions on Monday came after a number of other countries, mostly in Europe, halted their rollouts late last week.

The World Health Organization (WHO) has backed the use of the vaccine and said it has seen no evidence that the shot had caused clotting.

The UN health agency is reviewing the reports of possible side effects related to the shot and urged countries not to suspend vaccinations, as its top scientist said people should not panic.

The European Union’s medicines regulator – the European Medicines Agency (EMA) – said in a statement that it has not found any evidence of links between reported thrombosis cases and the AstraZeneca shot, saying that the shot’s benefits outweigh the risks and was safe to use.

The regulator is reviewing the shot and will issue a decision on any further action on Thursday, it said.

German Health Minister Jens Spahn said the country suspended the use of the shot on the advice of the national vaccine regulator, the Paul Ehrlich Institute.

The institute had called for further investigation into seven reported cases of clots in the brains of people who had received this vaccination.

“Today’s decision is a purely precautionary measure,” Spahn said.

France and Italy announced similar moves shortly afterwards.

French President Emmanuel Macron said the use of the AstraZeneca shot would be suspended as a precautionary measure until at least Tuesday afternoon when the European Union’s medicines regulator – the EMA – will issue its recommendation over the vaccine.

Macron did not elaborate on the reasoning behind the decision, but told a news conference he hoped France would be able to vaccinate with AstraZeneca shots again “soon”.

Italy’s medicines authority AIFA meanwhile said it was implementing its own suspension as a “precautionary and temporary measure” pending rulings by the EMA.

The announcement followed the seizure of hundreds of thousands of doses of the vaccine by Italian prosecutors in the northern region of Piedmont, where a teacher died following his vaccination.

Experts are investigating whether there is a connection between his death and the vaccination.

Late on Monday, Spain’s Health Minister Carolina Darias said the country was suspending its use of the vaccine for two weeks as a “precaution”.

She said the decision would remain in place until the EMA “analyses the recent incidences of blood clots, notably over the weekend”.

AstraZeneca has said there is no cause for concern with its vaccine, which is jointly produced with the United Kingdom’s University of Oxford, and that there were fewer reported thrombosis cases in those who received the shot than in the general population.

Peter Drobac of Oxford University told the AstraZeneca vaccine went through “rigorous clinical trials” and blood clots were not identified as a problem.

“The safety pause, I think it is certainly the prerogative of the regulators in these countries. However we have heard from the World Health Organization, the European Medicines Agency and others, that at this point the benefits of vaccination clearly outweigh the risks,” Drobac said.

The last time your blood pressure was measured, did you rush to the appointment while sipping coffee? Were you perched on an exam table with your legs dangling or your arm hanging? Were you chatting with a medical staff member or scrolling social media on your phone? Was your bladder full?

Any of these actions could throw off your blood pressure reading – in some cases, by as much as 33 millimeters of mercury (mmHg). That could leave you and your doctor unable to determine if you need treatment or whether any current treatment is working.

“The measurement of blood pressure is probably the most taken-for-granted measurement done across any clinical environment,” said Dr. Shawna Nesbitt, medical director at Parkland Health’s Hypertension Clinic and a professor in the department of internal medicine at UT Southwestern Medical Center, both in Dallas. But not enough attention is paid to its validity on a day-to-day basis, she said. “I think the routineness of it is why it is taken for granted – and basically done wrong probably 60, 70% of the time.”

Blood pressure is considered high in a teen or adult when the systolic (top number) is at least 130 mmHg, or the diastolic (bottom number) is 80 mmHg or more. Blood pressure guidelines issued by the American Heart Association and American College of Cardiology call for, among other things, readings to be taken with the patient seated in a chair with feet flat on the floor, their back supported and their upper arm cuffed and raised to heart level.

While it’s important to know how to properly measure blood pressure, it’s just as important to understand why the guidelines should be followed.

“Blood pressure is one of those factors that has a lot of variability. It’s all over the place,” said Dr. Paul Whelton, a renowned professor at the Tulane University School of Public Health and Tropical Medicine in New Orleans and president of the World Hypertension League, a nonprofit dedicated to the prevention and control of high blood pressure.

The key is to avoid factors that naturally cause a temporary increase in blood pressure.

“Even if you’re just rushing in from your car, that may generate some stress that increases blood pressure at least momentarily,” Nesbitt said. “What we want to measure is not that momentary increase. We want what is the most representative blood pressure on average for you in your regular life.”

Here are some key points to keep in mind to ensure an accurate reading so you and your medical team can determine the right treatment if your blood pressure is too high.

Plan ahead. It’s important to avoid exercise, smoking and caffeine at least 30 minutes before the blood pressure reading. All of these temporarily raise heart rate and blood pressure. Smoking also constricts blood vessels in the short term, altering the way blood flows through the body.

Keep calm. Ideally, you should relax for about five minutes before your blood pressure measurement. That’s because your body’s “fight or flight” response – officially called your sympathetic nervous system – automatically revs up in response to stress or physical activity. The heart accelerates and blood vessels tighten – ways the body prepares to react to perceived danger by fighting or running away.

Bathroom break. Once you arrive for your appointment, take a moment to visit the bathroom. “If your bladder is full, it sends a response to your brain that also stimulates the nervous system,” Nesbitt said. “And that increases blood pressure as well.”

Posture matters. Next, pay attention to how you’re sitting. “Be seated with your back supported, feet flat on the ground,” said Dr. Yvonne Commodore-Mensah, an associate professor at Johns Hopkins School of Nursing in Baltimore. Not following these steps could increase blood pressure from 5 to 20 mmHg, she said.

One reason: Even exertion that we barely notice, such as engaging muscles to sit up straight on an examination table, can affect blood pressure, Nesbitt said. Different postures also affect the way blood circulates. For example, crossing your legs at the knees compresses blood vessels, reducing the amount of blood returning to the heart, so the body has to work harder to keep the blood flowing, she said.

Shhhh. During the test, both you and the health care professional should be quiet. Talking can raise blood pressure even if you’re not discussing something that’s obviously stressful. Talking also stimulates the brain, Nesbitt said. “It’s not just the act of talking. It’s the thinking and the engagement and what you’re saying that also may drive blood pressure up.”

The effect of these factors on blood pressure is not minor: According to the ACC, talking can increase a measurement by up to 19 mmHg and crossed legs by up to 15 mmHg. If your arm is hanging, rather than supported at heart level, readings could be up to 20 mmHg too high. Lack of leg or back support can raise blood pressure by 5 mmHg, and a full bladder can increase it by up to 33 mmHg.

“If you do most of these steps wrong,” Commodore-Mensah said, “just think about the dramatic impact it can have on the blood pressure that is measured.”

If you measure your blood pressure at home – which she said is “absolutely critical” if you have hypertension – follow all of the guidelines and keep a record of the results to show your doctor. It is the best way to track high blood pressure long term. But there’s still a chance the device may still show a wrong measurement.

“There are a lot of instruments out there,” and most are labeled “FDA cleared,” said Whelton, who led the writing committee for the AHA/ACC blood pressure guidelines. “All ‘FDA cleared’ means is the component parts function as they’re supposed to. But it doesn’t tell you anything about how well or how poorly the device measures blood pressure.”

The Directorate General of Health Services (DGHS) has issued an advisory to all states and Union Territories on the rational use of cough syrups in children, following consumption-related deaths in Madhya Pradesh and Rajasthan.

In its communique, the DGHS advocates “judicious prescribing and dispensing of cough syrups for children,” arguing that most “acute cough illnesses in children are self-limiting and resolve without pharmacological intervention.” It further says that cough and cold medications should not be prescribed for children under two years. “These are generally not recommended for ages below five years and above that, any use should follow careful, clinical evaluation with close supervision and strict adherence to appropriate dosing, the shortest effective duration and avoiding multiple drug combinations,” the DGHS said. It highlighted the need for making everybody aware about adhering only to prescription by doctors.

As a first-line approach for children suffering from cough and cold, it has made a case for non-pharmacological measures, including “adequate hydration and rest.”

Since there have been reports that the deaths were caused by fake drugs, the DGHS has urged all healthcare facilities and clinical establishments to ensure “procurement and dispensing of products manufactured under good manufacturing practices and formulated with pharmaceutical-grade excipients.”

The advisory applies to health departments in all States/Union Territories, district health authorities, government dispensaries, primary healthcare centres (PHCs), community health centres (CHCs), district hospitals and medical institutions.

A multi-disciplinary team comprising experts from NCDC, NIV, ICMR, AIIMS Nagpur and state health authorities is investigating all possible causes behind the deaths. Although there have been reports of contamination and fake syrups, none of the samples tested so far contained diethylene glycol (DEG) or ethylene glycol (EG), contaminants that are known to cause serious kidney injury. The Madhya Pradesh State Food and Drug Administration (SFDA) also tested three samples and confirmed the absence of DEG/EG.

Further blood/CSF samples have been tested by NIV Pune for common pathogens. One case has been found positive for leptospirosis.

As regards the reports related to two deaths of children in Rajasthan because of contaminated cough syrup consumption, it has been clarified that the product in question did not contain propylene glycol, which can be a potential source of contaminants. Additionally, the product under reference is a dextromethorphan-based formulation, which is not recommended for paediatric use.
